What is Attendant Care?
Attendant Care services offer direct, hands-on assistance to individuals with intellectual and developmental disabilities, addressing their functional needs in daily activities. This service is provided to allow the individual to remain within their home, participate in their communities and avoid institutionalization.

USING YOUR WAIVER
1. Confirm Your Waiver
Make sure you have an active Medicaid waiver that qualifies you for services.
2. Choose Moseki Wellness Services (MWS)
Let your case manager know you’d like to receive services from Moseki Wellness Services.
Ask to “pick” Moseki Wellness Services as your provider. Sign the official pick list to confirm your choice.
3. We’ll Reach Out
Once we’re selected as your provider, Moseki Wellness Services will contact you directly to begin setting up your personalized services.
